L’endométriose et la fistule obstétricale touchent de nombreuses femmes et altèrent gravement leur qualité de vie, mais elles restent encore largement invisibles. Entraînant isolement social, souffrance physique et psychologique, notamment en Afrique, où les contraintes culturelles et économiques ont renforcé le silence autour d’elles. À la lumière de la normativité du vivant de Canguilhem, cet article met en exergue l’utilité de redéfinir le pathologique, comme incapacité à être « normatif », et intègre les expériences vécues des patientes. L’analyse montre les défis concrets qui entravent la reconnaissance de l’endométriose et de la fistule obstétricale. D’autre part, elle propose des stratégies émergentes pour promouvoir une gouvernance éthique et inclusive de ces pathologies intégrant le protocole médical et l’expérience subjective des malades. L’objectif en liant savoir médical, réflexion philosophique et vécu des patientes est de dépasser la vision purement mécaniste de la maladie, et de proposer une approche normative inclusive de la santé reproductive à travers un changement de paradigme.

FROM STIGMATIZATION TO BROKEN SILENCE: FOR ETHICAL GOVERNANCE OF ENDOMETRIOSIS AND OBSTETRICAL FISTULA BASED ON THE WORK OF GEORGES CANGUILHEM

Abstract:

Endometriosis and obstetric fistula affect many women and severely impair their quality of life, yet they remain largely invisible. This leads to social isolation, physical and psychological suffering, particularly in Africa, where cultural and economic constraints have reinforced the silence surrounding these conditions. Drawing on Canguilhem’s concept of the normativity of life, this article highlights the importance of redefining pathology as an inability to be "normative," and incorporates the lived experiences of patients. The analysis reveals the concrete challenges that hinder the recognition of endometriosis and obstetric fistula. Furthermore, it proposes emerging strategies to promote ethical and inclusive governance of these conditions, integrating medical protocols and the subjective experience of patients. By linking medical knowledge, philosophical reflection, and the lived experiences of patients, the aim is to move beyond a purely mechanistic view of disease and to propose an inclusive, normative approach to reproductive health through a paradigm shift.
